As evidenced by my signature below, I acknowledge that the cancellation procedures and dates have been explained to me and I understand the following:

1) My membership will be cancelled 30 days after the date above,

2) I will be billed for the 30 days following the Cancellation Date,

3) Any unused or outstanding classes are forfeited and fully earned by Illuminate Sweat and

4) If I purchase an Illuminate Sweat Membership in the future my price may be higher, particularly if

my cancelled membership was at a presale price.

5) (If Applicable) I understand due to the terms of my membership, I will be responsible for the

remaining totals on the account to be paid no later than the expiration date of classes above.
